We are able to accept same day or emergency referrals although planned placements are preferred. Information on each child/young person is required, to ensure we can effectively meet their needs. This LAC documentation should consist of previous review reports, observations on general behaviours, Risk Assessments, Care Plans and the completion of our own Referral Pack which will include a Placement Agreement stating the intended place of address for the child/young person.
It is hoped that the young person will visit the home with the Social Worker and/or family members to view their potential, new home and learn about the service we offer and to minimise anxiety or stress. Admissions should be arranged in a planned manner that is beneficial to the individual needs of the child/young person.
All placing authorities will be expected to arrange and attend a Planning Meeting to develop/update the Care Plan within 72 hours for an emergency placement and 10 days for a planned placement, following the child/young person’s admission to the home.
The first 12 weeks of the placement will be an assessment period. This is to ensure that the child or young person is appropriately placed and the home can effectively meet their needs.
Further Planning Meetings or Placement Reviews will be held at 12 weekly intervals following the initial meeting in order to review the placement to ensure that we are offering a service that will provide the best outcomes for the child/young person.
